The oculomotor nerve is tested by examining the function of the extraocular muscles that it innervates (LPS, SR, MR, IO, IR) as well as the ciliary body and constrictor pupillae. To test the sympathetic and parasympathetic innervation that runs with the nerve, the examiner should check for papillary dilation and constriction reflexes. Parasympathetic innervation may also be tested by checking for pupillary accommodation.
Oculomotor nerve damage affects both accommodation and the pupillary light reflex, whereas CNS disease such as neurosyphilis can result in a pupil that constricts on accommodation but not in response to light (known as Argyll-Robertson pupil). Argyll-Robertson pupil is also known as the prostitute’s pupil, one that accommodates but does not react [to light].
