trigeminal nerve divisions (pins)
Back
Forward
blue pin = V1 (ophthalmic division)
green pin = V2 (maxillary division)
white pin = V3 (mandibular division)

V1/ophthalmic nerve-
The ophthalmic division of the trigeminal nerve (V1) is tested by stimulation of the skin of the forehead. A corneal reflex (a consensual reflex) can also be elicited – if the reflex is not present V1 injury may be suspected. If the test elicits a reflexive blink on only the contralateral side, a facial nerve (CN VII) problem may be suspected (recall that the facial nerve gives rise to branches which innervate the orbicularis oculi).

V2/maxillary nerve-
To test the maxillary division of the trigeminal nerve (V2), the examiner checks for sensation of the skin overlying the bony cheek.

V3/mandibular nerve-
To test the sensory function of V3 (mandibular division of the trigeminal nerve), the examiner stimulates the skin overlying the chin (testing mental nerve) and the side of the cheek (testing buccal nerve). Testing for general (not special) sensation over the anterior 2/3’s of the tongue also tests V3. To test the motor branch of V3 the examiner places his/her hands over the patients masseters and asks them to clench their teeth. To test motor innervation to the pterygoid muscles and the superficial masseters the examiner pushes the patient’s jaw to the left and/or right while the patient resists.
