The bloodless fold of Treves arises from the anti-mesenteric border of the ileum and passes across the caudal portion of the ileocecal junction to join the mesentery of the appendix. It serves as a useful landmark for locating the appendix in situ.
Pain resulting from appendicitis is referred to the umbilicus (also innervated by T10 spinal segment). As the overlying peritoneum becomes agitated and inflamed, the pain becomes better localized to the lower right quadrant of the abdominal region.
McBurney’s point is located at the junction between the lateral 1/3 and the middle 1/3 along a line traced from the ASIS (anterior superior iliac spine) and the umbilicus. Tenderness at this location is a strong indicator of acute appendicitis.
